Your healthcare needs are different as you age. The services at our Senior Health Centers are specifically designed to meet the changing needs of older adults and to help you experience the best in healthy aging.
Each team member has a personal desire to serve and work with older adults – from the very well to the very frail. Each member believes older adults need and deserve more attention. This philosophy of care stresses wellness and health maintenance to support the individual's efforts to live as independently as possible.
Clinical services have been created in an environment that is conducive to individualized care, treating the whole person and devoting the time needed to deal with the sometimes complicated issues older adults experience. The specialized team is provided by Northwest Health System and has access to the entire array of services the System offers.
How We Customize Clinical Services to Meet Your Needs
- A multidisciplinary team composed of fellowship-trained, board certified geriatricians, nurse practitioners, nurses, social workers, neuropsychologists and ancillary staff specially trained in the care of older adults
- Continued management of patients in many health care settings common to older adults: primary care clinic, hospital inpatient, home visits, nursing home, and home health care
- Case management that identifies resources and coordinates community services to aid patients and their families in all settings
- Memory evaluation services to provide diagnosis and assist with management of memory disorders. Primarily consults and works collaboratively with the patient's primary care physician. Support and educational services are offered to family and caregivers.

- Consultation services including comprehensive assessment of older adults for rehab potential, recommendations for maintaining independent residence, and fall-risk assessment
- Support for family and caregivers facing age-related functional decline of an older adult
